Player/Parent Assumption of Risk, Release and Waiver of Liability, and Indemnity Agreement 

The undersigned and their participating children fully understand and appreciate both the known and potential dangers of participating in the events of the Philadelphia Liberty Girls Baseball Club and the Philadelphia Liberty. The undersigned, on his or her behalf, and on behalf of such participating children, here, by releases, waives, discharges, and covenants, not to sue the Philadelphia Liberty Girls Baseball Club, the Philadelphia Liberty, its directors, officers, employees, volunteers, and agents from all liability to the undersigned or such participating children for any loss or damage, and any claim or demand on account of any property damage or any injury to, or an illness, or the death of, the undersigned of such participating children. The undersigned further expressly agrees that the foregoing assumption of risk, release, and waiver of liability, and indemnity agreement is intended to be broad and inclusive, as is permitted by the laws of the state of Pennsylvania, and that if any portion thereof is invalid, it is agreed that the balance shall, not withstand continue in full legal force and effect.

I have carefully read and voluntarily signed this assumption of risk, release, waiver of liability, and indemnity agreement. I further agree that no oral representations, statements, or inducement, apart from the foregoing written agreement, have been made. I know that by agreeing to this agreement, I am giving up valuable legal rights, including the right to recover damages from the Philadelphia Liberty Girls Baseball Club and the Philadelphia Liberty. I understand that this document is a promise not to sue and a release of and indemnification of all claims. If signing on behalf of a minor, I also understand that this agreement is made for my minor child and/or legal words. I represent and warrant to the Philadelphia Liberty Girls Baseball Club and the Philadelphia Liberty. I have full authority to sign this agreement on behalf of a minor.

Media release form and waiver

I hereby give permission to the Philadelphia Girls Baseball Club, the Philadelphia Liberty, and it's volunteers, officers, agents, and employees to use photographs, voice recordings, or video taken of my child (if registering a minor) or myself during the games and events associated with the Philadelphia Girl Baseball Club and the Philadelphia Liberty in any manner to help promote the organization’s activities as determined in the sole discretion of the Philadelphia Liberty Girls Baseball Club, and the Philadelphia Liberty. Such use could include publications, media releases, announcements, electronic or otherwise, and on organizations, websites, or social media pages. I understand that neither I nor my child will receive any compensation if such images or videos appear in any manner.
